Dear Building Manager/Staff,
A large number of older adults with dementia live in the community. The City of Chicago Age
Friendly Commission and multiple building management agencies are partnering with the
University of Illinois at Chicago to create dementia friendly older adult buildings, by offering
training to building managers and staff designed to help them address the complex needs of
residents with dementia. The goal of the “Dementia Friendly Buildings” training project is to
improve the ability of building management staff to understand dementia and its presenting
features in order to enhance the support provided to older adults with dementia.
You are invited to participate in this brief research survey after the webinar because you are a building manager or staff of a high rise building in Chicago which houses older adults and you have attended the “Dementia Friendly Buildings” training program. The purpose of this research survey is to obtain your opinion about the “Dementia Friendly Buildings” training program and make improvements to our program to ensure that it is responsive to the building managers and staff needs so that they are able to better support the residents.
We are asking you, as a participant, to provide thoughtful and honest responses to the survey. All information collected is confidential. Electronic data will be kept in a secure server in the university’s network. The data gathered will be shared and reported in aggregate and may be published and shared externally.
The risks to you are minimal and include the time it will take you to complete the survey. Although you will not benefit directly from participating, the information you provide will contribute towards guiding program refinement.
Participation in this survey and the post-webinar survey is voluntary. Some of the survey questions will ask you to disclose sensitive information, including gender identity. You may skip any questions that you do not wish to answer. Please note that, if you choose not to complete this survey, your decision will in no way impact your position with your employer.
This survey and related training and research are independently developed and administered by the study investigators.
